Federal prosecutors have charged a man from Bay View in the theft of a wide-screen television from the Zablocki Veterans Affairs Medical Center. 

Surveillance video showed Jeffrey Polak, 45, dressed as a maintenance worker, walk into the VA with a dolley and then clamly walk out with a 52-inch Samsung LCD TV.

In a sworn affadavit, federal investigator Jeffrey Walsh said a detective "received a phone call from a person who identified himself but wished to remain anonymous" and "said he recognized the individual as a relative of his, Jeffry Polak."

"He said that...Polak's father had told him that Jeffry had just gotten a big screen television."

When investigators interviewed Manfred and Erma Polak, Manfred "said that he had seen the television footage and immediately knew it was his son, Jeffry because he recognized his face, body, the way he walked and also the two-wheeled cart."

"Manfred said the cart was his and that Jeffry had borrowed it a couple of days before the television was stolen."

Polak faces charges of theft of government property, which is punishable by a maximum of 10 years in prison and $250,000 in fines.
