4. Prize is non-transferable and cannot be exchanged for cash or any other prize/merchandise.
5. Odds of winning are dependent upon the number of entries received. Neither Journal Broadcast Group nor any affiliate of Journal Communications, Inc., Amazing Siding & Windows will not be responsible for lost, late, mis-directed, or illegible entries.
6. Parent or guardian of child chosen must complete a consent form, and provide child’s name, address, city, state, zip code, home number, birth date and social security number. Any other information given is done voluntarily.
7. Acceptance of prize constitutes permission to the Sponsors, radio station, and their agencies to use the winner’s name and/or likeness for the purpose of advertising and trade without further compensation.
8. By accepting prize, winner agrees to hold sponsors, their respective directors, officers, employees and assigns, harmless against any and all claims and liability arising out of acceptance or use of prize. Winners assume all liability for any injury or damage caused, or claims to be caused, by participation in this promotion or use or redemption of prize.

10. Contest runs February 11, 2009 – February 11, 2010.
11. Parent or guardian of winner agrees to sign publicity and release form upon acceptance of prize and is responsible for all taxes and fees associated with prize. Parent or guardian of any potential winner must sign an affidavit of eligibility/prize acceptance form and release of liability within 14 days of notification or upon acceptance whichever is sooner. Non-compliance with this time period may result in forfeiture of prize and an alternate winner may be selected.
12. Prize winners will be contacted by email. Participants will agree on a mutually agreeable day to participate. WTMJ Radio reserves the right to choose an alternate winner if there is no response to the initial request.
13. WTMJ reserves the right to substitute a prize of equal or greater value if the prize becomes unavailable.
14. WTMJ-AM reserves the right to end this promotion or vary prizes at any time without prior notice.
