Last evening, Texas Rangers outfielder Josh Hamilton tossed a foul ball into the stands. While going for the ball, a spectator lost his balance and fell 20 feet to his death. The man's eight-year old son was standing next to him as he went over the railing.

I'm told that Major League Baseball discourages players from throwing balls into the stands under circumstances like this - but I see balls thrown into the stands at the end of almost every inning.
So, was this simply a tragic accident - or an accident waiting to happen?
